Central United Life Health carriers are asked to provide a healthy lifestyle rebate to those members who are enrolled in and maintain participation in a health wellness, maintenance or improvement plan approved by the Company.

The firm is available to policyholders in Florida who are covered under a major medical or hospital plan underwritten by their firm or assumed by Central United. There are rebates available to policyholders who achieve results which indicate a healthy lifestyle. The rebate is reflected as a percentage of premium collected during the calendar year.

The company reserves the right to change the rebate program and will notify eligible program participants upon such changes. The Company must obtain the Florida Office of Insurance Regulation’s approval of any changes prior to implementation.
